Reporting forms
TESSA online reporting forms
TESSA is our online reporting system. Scheme participants use the online reporting forms in TESSA to meet their reporting obligations. The TESSA for scheme participants page has more information, including instructional videos for each reporting form.

Detailed Scope of Works form
We provide auditors with the DSW Submission Form – Scheme Participants. Auditors submit this form as their Detailed Scope of Works (DSW) for auditing scheme participants. The auditor submits the DSW to us for approval before commencing the audit.

Summary of Liable Acquisitions template (voluntary)
We no longer require scheme participants to use the Declaration of Liable Acquisitions. We provide the Summary of Liable Acquisitions excel template for voluntary use.
